Lori Leopold, MSW, Patient Care Manager
Lori Leopold is a dedicated social work professional with over 30 years of experience supporting individuals and families through life’s most challenging moments. While earning her Master’s degree in social work, she wrote and published a comprehensive guide to help parents and caregivers navigate the complex and ever-evolving landscape of resources for children with special needs. Lori brings extensive expertise in hospice, palliative care, bereavement, and home health. Most recently, she served as a medical social worker with VNA Health, providing compassionate support to patients and families across these disciplines. Prior to that, she worked as a hospice medical social worker in Los Angeles, ensuring patients received dignified, holistic care at the end of life. Lori is known by many names; mom, sister, social worker & friend, but by far her favorite title is “Nana” to her 6 grandchildren.
